Additional Support

For Moms

Through the Moms program, care managers assess members using the Edinburgh Postnatal Depression Scale. Care managers work with members to identify symptoms; determine their level of understanding about “baby blues” versus postpartum depression; identify prior history of depression; recommend types of support systems; monitor medication adherence; and provide member education. Care managers also provide members with community resources and work with them to develop a plan to address their goals and identify clinical needs. The Moms program is available at no cost to women diagnosed with behavioral health needs during pregnancy through those who are two years’ postpartum, as well as women who lost a pregnancy.

The program is available to State Health Plan primary members ages 18 and older. Ask your network provider for more details.
